Job Description:

As a Patient Placement Specialist at GCPC (Gulf Coast Placement Center ), your role is crucial in facilitating efficient and timely healthcare services to patients. You will be responsible for coordinating and managing patient admissions, transfers, and discharge processes within our network of healthcare facilities. Your primary objective is to ensure that patients are placed appropriately based on their medical needs and the availability of resources.

Responsibilities:

1.    Patient Placement: Assess incoming patient information, including medical condition, required level of care, and specific facility requests, and determine the most suitable placement within the GCPC network.

2.    Collaboration: Communicate and collaborate with healthcare professionals, including physicians, nurses, and administrative staff, to gather relevant patient information and facilitate the placement process.

3.    Facility Coordination: Liaise with various healthcare facilities within the network to identify bed availability, specialty services, and any specific patient requirements.

4.    Admissions and Transfers: Coordinate the admission and transfer process for patients, ensuring a smooth transition and appropriate documentation.

5.    Resource Optimization: Strive to optimize the utilization of available healthcare resources, including beds, equipment, and specialized services, to meet the needs of patients.

6.    Communication: Maintain effective communication channels with patients, their families, and referring healthcare providers to keep them informed about placement status and any updates.

7.    Compliance: Adhere to all applicable legal, ethical, and regulatory guidelines, including patient privacy and confidentiality laws.

8.    Data Management: Accurately enter and maintain patient information and placement details in the GCPC database, ensuring data integrity and confidentiality.

9.    Problem Resolution: Act as a point of contact for any placement-related issues, address concerns promptly, and collaborate with relevant stakeholders to find suitable solutions.

10.   Continuous Improvement: Contribute to process improvement initiatives by identifying areas for enhancement in patient placement procedures and sharing insights with the team.

Requirements:

1.    Education: High school diploma or equivalent. Additional training or certification in healthcare management or a related field is preferred.

2.    Experience: Prior experience in a healthcare or call center environment is beneficial, especially in roles involving patient care coordination, admissions, or transfer processes.

3.    Knowledge: Familiarity with medical terminology, healthcare facilities, and different levels of care is advantageous.

4.    Communication Skills: Excellent verbal and written communication skills to effectively interact with patients, healthcare professionals, and team members.

5.    Organizational Abilities: Strong organizational and multitasking skills to handle a high volume of patient placements and prioritize tasks efficiently.

6.    Problem-Solving: Ability to think critically and make informed decisions in time-sensitive situations, considering patient needs and available resources.

7.    Technology Proficiency: Comfortable using computer systems, databases, and software applications for data entry and retrieval.

8.    Empathy: Compassionate and empathetic approach towards patients and their families, understanding the stress and emotions associated with healthcare decisions.

9.    Adaptability: Flexible and adaptable to changing situations, able to work under pressure, and maintain composure in a fast-paced call center environment.

10.   Compliance: Knowledge of healthcare regulations, including HIPAA, and commitment to maintaining patient privacy and confidentiality.
